Langerhans cell histiocytosis (LCH) is a rare atypical cellular disorder characterized by clonal proliferation of Langerhans cells leading to myriad clinical presentations and highly variable outcomes. There is a paucity of Indian studies on this subject. This is a retrospective observational study of patients with LCH who presented at the Tata Memorial Hospital between January 1987 and December 2002. Fifty-two patients with LCH were treated in the study period. Due to the long observation period and variability in diagnostic and therapeutic protocols, the patients were risk-stratified based on present criteria. The disease pattern, management approaches and treatment outcomes of patients were recorded. Statistical analyses were done using Student's 't'test, test for proportion and survival estimates based on the Kaplan-Meier method. The median age at presentation was 3 years and more than 48% of the patients had Group I disease. Skeleton, skin and lymphoreticular system were the commonly involved organs. Majority (80%) required some form of therapy. The projected overall survival is 63% at 10 years and mean survival is 118 months. Seventeen percent of surviving patients developed long-term sequelae. The clinico-biologic profile of LCH patients in India is largely similar to international patterns except a higher incidence of lymphoreticular involvement. Majority of the patients respond favorably to therapy and have a good outcome, except a subset of Group I patients who warrant enrolment in clinical trials with innovative therapeutic strategies to improve outcome.

Though the role of anaesthesiologist is crucial, the public knowledge of anaesthetic practice is limited. It is thus important for us to think that why the importance of this speciality is not known and how to make people aware about the vital role played by an anaesthesiologist in medical set up. The present study was conducted in a tertiary care teaching hospital, on 150 adult surgical patients posted for elective surgery to assess patient's knowledge about the role of anaesthesiologist, their preoperative concerns related to anaesthesia, post operative complaints and level of satisfaction regarding anaesthesia services. A majority (92.67%) of patients felt that anaesthesia was necessary for surgery, only 42.67% knew that it was given by an anaesthesiologist. Only 27.33% of the patients knew that besides anaesthetizing, anaesthesiologists monitors & takes care of vita! signs throughout surgery. Patients were not much aware of their role in ICU (7.33%), painless labour (12.67%) & pain clinic (4.67%). Only 15.33% patients had knowledge about anaesthesia risks given in consent form. Though people with higher literacy levels had greater knowledge about anaesthesia compared with the lower literacy group (P<0.001), even their mean scores fell in average range. The statistical association between past exposure to anaesthesia and knowledge about anaesthesia was not significant (P>0.1). Contribution of anaesthesiologists in patients knowledge was the least (1.33%). The number of patients having preoperative fears was much more (61.33%) than the actual number of patients having discomfort during surgery (16.67%). Large majority of patients were satisfied with the quality of anaesthesia services (99.33%), and showed willingness to know about their anaesthesiologist in case of any future surgery (70%). Concludes that there is a widespread misconception amongst the public about the role of anaesthesiologists inside and outside the operating room, however services provided were considered satisfactory. Therefore, sufficient time should be given to educate the patients about anaesthesia and the various role an anaesthesiologist plays. Meeting during the preanaesthetic examination should be used to fulfill this purpose.

J parasitic Dis 2007, 31(2), 128-33.
The use of indoor residual spray (IRS) and insecticide-treated bed nets (ITBN) is the most important chemical control tool for vector control, and thereby interrupting the transmission of malaria and other vector borne diseases. In ten tribal dominated malarious Primary Health Centers of East Godavari district of Andhra Pradesh, 134 human dwellings of 15 villages were searched. Satisfactory IRS with malathion was done in only 11.2% houses. Of the 303 living rooms examined, IRS was done only in 13.2% rooms and 76.1% verandahs. Refusal rate in living rooms was noted as 86.7%. This was mainly due to lack of knowledge regarding the importance of IRS and reluctances among villagers to remove domestic articles and dislike of the smell of malathion. Techniques of IRS were found unsatisfactory during concurrent supervision. Only 40% tribal villagers were using ITBN at night. A 60% ITBN retention rate was noted. Out of nine anopheline species in the study area, three vectors of malaria, i.e., Anopheles culicifacies,An.fluviatilis and An. annularis were noted in both human dwellings and outdoors. Poor coverage (13.2%) and high refusal rate (86.7%) of IRS with malathion along with high retention rate (60%) among ITBN users are the main causes for increasing trend of malaria. This is due to lack of knowledge and motivation in receptor population. High density of three vector species in outdoor sampling was noted.

In anaesthetic practice, exposure to a hazardous substance should be eliminated or controlled where practicable. Over the past few years, the use of nitrous oxide has become debatable. To assess the current anaesthetic practice in India on the use of nitrous oxide, a total of 400 questionnaires constituted by 15 questions on three topics- use of nitrous oxide, medical air and TIVA(total intravenous anaesthesia)- were distributed. Anaesthesiologists were selected randomly and requested to fill up the questionnaire. Of the 400 questionnaires, 40 questionnaires were discarded due to incomplete answer, leaving a total of 360 valid returned questionnaires, a 90% response rate. 27.22% respondents stated that the use of nitrous oxide had decreased. Whenmedical air was available, 66.67% felt that they would use it frequently. A total of 214 (59.44%) stated that theyused total intravenous anaesthesia occasionally. The results showed that although anaesthesiologists had reduced their use of nitrous oxide, this was due to medical considerations rather than concerns over health and pollution issues arising from the use of nitrous oxide.

The study was undertaken to evaluate the hepatoprotective effect of curcumin and silymarin alone and in combination, against aflatoxicosis in male broiler chicken. Liver damage was assessed by quantifying the serum levels of Total Protein, Alanine aminotransferase (ALT) and Aspartate aminotransferase (AST) supported by histopathological examination. Aflatoxin administration at 1 ppm level resulted in increase in serum ALT and AST, decrease in total serum protein with varying degrees of liver damage ranging from diffuse vacuolar degeneration of hepatocytes, necrosis and tendency for fibrosis. Curcumin (10mg, 20mg) and silymarin (500ppm, 1000ppm) produced dose dependant improvement in all the parameters studied and they were more or less similar in their hepatoprotective effect. The combined effect of both the drugs, Curcumin 10mg + Silymarin 500ppm and Curcumin 20mg + Silymarin 1000ppm, produced better results in a dose dependant manner indicating the synergistic effect of the two flavonoids.

Spiral embedded tubes are routinely being used in anaesthetic practice these days as a preventive measure in cases where kinking of the endotracheal tube is anticipated. In spite of this advantage spiral embedded tubes can at times lead to disaster since any deformity in these tubes is permanent and leads to occlusion of a patent airway. The study was planned to-.evaluate the clinical efficacy and safety of Gasex syrup in functional dyspepsia. Dyspepsia is a symptom complex of upper abdominal pain or discomfort believed to originate in the upper gastrointestinal (GI) tract. The pathogenesis of functional dyspepsia remains uncertain. However, regardless of the cause, the treatment effects have generally not been impressive. The study was an open clinical trial, conducted as per the ethical guidelines of Declaration of Helsinki. All patients suffering from pain or discomfort centered in the upper abdomen with associated bloating, belching, nausea, and heartburn were included in the study, patients having severe vomiting and diarrhea, and those suffering with gastric or duodenal ulcer, cancer or varices disease were excluded from the study. A thorough history, symptomatic evaluation and clinical examination was done for all patients before treatment and during follow-up visits every week till the end of treatment on Day 28 along with recording the occurrence of any adverse event/s. The predefined primary endpoints were rapid symptomatic relief from upper abdominal pain, heart burn and bloating. The predefined secondary endpoints were short- and long-term safety, and overall compliance to the drug treatment. A total of 50 patients were enrolled into the trial and all the patients completed the study. A significant reduction (p<0.0001) in the mean symptom score of upper abdominal pain, heartburn, abdominal bloating, belching, fullness of stomach after meals, and nausea were observed after 28 days of treatment with Gasex syrup. There were no clinically significant adverse events, either reported or observed, during the entire study period. Therefore, it may be concluded that Gasex syrup is clinically safe and effective in the management of functional dyspepsia.

NSAIDs (non-steroidal anti-inflammatory drugs) are one of the most widely prescribed class of drugs in rheumatology and the gastrointestinal complications are the major cause of mortality and morbidity among the consumers. Hence, COX-2 selective inhibitors were developed with assumption of better safety profile (renal and gastrointestinal) than non-selective NSAIDs. However, recent evidences of cardiovascular and renal complications with their use have raised a question on their status in rational therapeutics. Moreover, recent reports from population-based studies indicated increased risk of myocardial infarction, death, and recurrent congestive cardiac failure in patients prescribed NSAIDs or rofecoxib than celecoxib. At present, the logical therapeutic approach is to prescribe NSAIDs at the lowest effective dose, for short duration, and without concomitant use of corticosteroids and anticoagulants. Pro-drugs, NO-NSAIDs having a nitric oxide moiety linked to a conventional NSAID, a competitive inhibitor of 5-lipoxygenase (5-LOX) and COX and combining COX-2 inhibitors with essential fatty acids (that lead to the formation of cardioprotective lipoxins and resolvins) are some of the options under trial.

At the 13th week (groups 5-7), these changes were more prominent. In higher dose group (EO @ 750 ?g/kg), swelling of endothelial cells, thickening of blood vessel walls, degeneration and fibrous tissue proliferation were also observed. The extent and severity of ovarian damage were found to be dose and time dependent, suggesting that the higher dose and prolonged duration of oestrogen administration successively, may cause more cytotoxic changes in the ovary. The standard cytotoxic doses of oestrogen were assessed to be 500 to 750 μg/kg, orally weekly for a period of 12 weeks.

Seroma formation after mastectomy typically delays recovery and adds to morbidity. This retrospective review was undertaken to identify factors which predict development of seroma after mastectomy for breast cancer patients. 119 consecutive patients intended for mastectomy for the treatment of primary breast cancer were included. Factors taken into consideration were epidemiological, peri-operative in nature and those related to wound drainage output. Total mastectomy was performed and axillary sampling was taken. All patients were reviewed within two weeks after leaving hospital, unless seroma formation was detected before discharge. The diagnosis of seroma was made clinically when a collection was detected beneath the skin flaps. Student's t test was used with continuous variables and the X2 test for categorical situations. Fisher's exact test was applied when small numbers were encountered. A two-tailed test of P<0.05 was considered significant. Univariate analyses were performed. The incidence of seroma formation was eight per cent. Five factors were identified to be significantly related to seroma formation: i) age over 45 years; ii) hypertension; iii) total drainage output exceeding 500 ml in the first three postoperative days; iv) drainage for more than eight days. Immediate breast reconstruction prevents the formation of seroma. The presence of hypertension in a patient over 45 years should alert the surgeon to possible seroma formation, particularly when the post-operative drainage exceeded 500 ml in the first three days. Appropriate preventive measures should then be implemented.

Sepsis and SIRS (systemic inflammatory response sydrome) is the most common cause of acute adrenal insufficiency. Several studies have been published in western literature on the incidence and outcome of acute adrenal insufficiency in patients with septic shock. As very little Indian data is available on this aspect, this study was carried out to (a) assess the integrity of HPA axis, in patients with septic shock, and (b) to study the cortisol levels in survivors and non-survivors. Twenty-eight consecutive patients admitted in medical ICU with diagnosis of septic shock were studied. Septic shock was defined as per ACCP/SCCM consensus conference criteria. Illness severity at admission was assessed by APACHE-II scoring system. Venous blood samples were taken for estimation of stress cortisol levels at admission. A random cortisol level of ≥ 15 μgm/dl was taken as normal. Twenty-eight patients (16 men and 12 women) were studied. The primary diagnoses were pneumonia (10), gastroenteritis (4), acute pyelonephritis (4), acute cholecystitis (2), pelvic abscess (4), and cellulitis (4). Six patients (2 women and 4 men) succumbed to the illness. The APACHE-II score was higher in non-survivors (n = 6) (28.33 ± 2.08) compared to survivors (n = 22) (19.73± 1.56). The mean random cortisol was the same in both groups (32.0 μg/dl ± 8.24). HPA axis is activated in septic shock. None of our patients studied had absolute adrenal insufficiency. There was nc significant difference in the cortisol level of survivors and non-survivors.

A simple and cost effective spectrophotometric method is described for the determination of torsemide in pure form and in pharmaceutical formulations. The method is based on the formation of blue colored chromogen when the drug reacts with Folin-Ciocalteu (F-C) reagent in alkaline medium. The colored species has an absorption maximum at 760 nm and obeys beer's law in the concentration range 30 - 150 μg mL-1. The absorbance was found to increase linearly with increasing concentration of TSM, which is corroborated by the calculated correlation coefficient value of 0.9999(n=8). The apparent molar absorptivity and sandell sensitivity were 1.896x103 L mor-1 cm-1 and 0.183 μg cm-2, respectively. The slope and intercept of the equation of the regression line are 5.4xl0-3 and l.00xl0-4 respectively. The limit of detection was 0.94. The optimum experimental parameters for the reaction have been studied. The validity of the described procedure was assessed. Statistical analysis of the results has been carried out revealing high accuracy and good precision. The proposed method was successfully applied to the determination of TSM in pharmaceutical formulations.

Role of psychological psycho behavioural factors may be equally important in the pathogenesis of coronary heart disease (CHD). An ex post facto study was conducted to investigate the role of Type A behaviour pattern and stressful life events in pathogenesis of coronary heart disease. Jenkins Activity Survey and Presumptive Stressful Life Events Scale were administered on 200 subjects (100 CHD patients and 100 healthy controls) of 30-55 years age group. Data were statistically analysed by using t-test. The patients and healthy controls significantly differed on Type A behaviour pattern and stressful life events scores of whole life and last one year. Further, the comparisons were made amongst the diffeent groups to establish the role of behaviour patterns in mediating the effects of life events in CHD. Results indicate that Type A behaviour pattern and presence of the stressful life events were found to be independent precursors to onset of CHD.

At 125 mg/kg po, it showed a mild anti-stress activity. Under in vitro 1,1-diphenyl-2-picrylhydrazyl (DPPH) free radical scavenging assay and lipid peroxidation assay the extract considerably inhibited, in a dose-dependent manner, the levels of DPPH free radicals and thiobarbituric acid reactive substances, respectively thus showing significant antioxidant property. The results suggested that it could be used for the treatment of oxidative stress-induced disorders.

Headache is one of the most common maladies to affect humans beings. 76 % of women and 57% of men report at least one significant headache per month; more than 90% experience at least one noteworthy headache in their lifetime. Study the profile of headache of patients attending a hospital located in rural surroundings. This will help in identifying the aetiology of the malady and hence relief measures. Subjects include patients more than 12 years age and with complaints of headache for more than 3 months attending the out-patients department or indoor at Kasturba hospital, Sewagram, Wardha. Patients underwent thorough clinical examination and psychiatric evaluation and clinical investigations were done as and when required. Majority of patients (n = 202) had primary headache (92.5%) which included tension type (47%), and migraine (44%). Secondary headaches constituted 7.5% of the total patients, and these include post-traumatic headache, refractive error, central nervous system lesions, cervical spondylosis, trigeminal neuralgia, and hypertension, in the order of descending frequency. Primary type headaches were common among housewives (40.1%), married persons (69.3%), those belonging to socioeconomic class II (47%), and in nuclear families (75.25%). Education does not have any significant role to play. Secondary headache profile could not emerge out of this study due to the small sample size.

Investigated the prevalence of parasitic infections in hospital patients having gastrointestinal complaints. The study covered a three-year period (January 2001-December 2004). Stool samples were collected from 485 patients (240 males and 245 females) who reported at Central Hospital, Benin City, Nigeria. Each stool sample was examined microscopically for the presence of ova and cysts, using iodine and formol-ether concentration methods. Overall, 43 (8.86%) stool samples were positive for intestinal parasites, with the rates of infection in the following decreasing order: hookworm (41.86%), Ascaris lumbricoides (32.6%), Entamoeba coli (6.97%), E. histolytica (6.97%), Trichuris trichiuris (6.97%), Taenia solium (4.65%) and Giardia lamblia (2.32%). Patients within the first four decades of their life had the highest prevalence of infection. All the mixed infections were found in females. Although females (10.6%) were more infected than the male patients (7.08%), the difference was not statistically significant (χ2=1.56; p > 0.05). The poor sanitary conditions within several areas of the city, coupled with inconsistent approach of government's de-worming campaign programmes, may be responsible for the continuous presence of helminthic and protozoal infections.

Pancreatic cancer is one of the most lethal human cancers and continues to be a major unsolved health problem as we enter the 21st century. This is the case despite advances in imaging technology and surgical management. Indeed, 80-90% of pancreatic cancers are diagnosed either at the locally advanced stage or metastatic stage. Cutaneous metastases originating from pancreatic cancer are relatively rare. The most common site of cutaneous metastasis is the umbilicus, and it is known as the Sister Joseph's nodule. Very few patients have been reported with cutaneous lesions disclosing pancreatic carcinoma at sites other than the umbilical area. To our knowledge, there were no previous reports on cutaneous pancreatic metastasis in Egypt. This is a report of a patient with cutaneous pancreatic metastases at the neck, followed by a review of reported non-umbilical cutaneous metastases from pancreatic carcinoma in the literature.

Oriental J Chem 2007, 23(2), 635-9.
7-Chloro-6-fluoro-benzothiazol-2-yl-amine was synthesized from 3-Chloro-4-fluoro phenylamine, which on reaction with hydrazine hydrate in a modified microwave oven containing a reflux condenser, magnetic stirrer, and thermostat control yielded 7-chloro-6-fluoro-benzothiazol-2-yl-hydrazine as the intermediate that is ultimately converted to 7-chloro-6-fluoro-benzothiazolo[2,3-d]-1,2,3,4-tetrazole by cyclization with sodium nitrite. Various derivatives were prepared by replacing the chlorine atom of 7-chloro-6-fluoro-benzothiazolo[2,3-d]-1,2,3,4-tetrazole with benzamideandalkoxides. Oxidation of the sulphur of the benzothiazole moiety resulted in sulfone and sulfoxide derivative of benzothiaozolo-tetrazole, which were subjected to chlorine substitution to yield the sulfone and sulfoxide derivatives of 7-substituted-6-fluoro-benzothiazolo[2,3-d]-1,2,3,4-tetrazole. The synthesized compounds have been characterized by analytical and spectral studies (IR and 1H NMR) and screened for their antimicrobial activity. The yield of the compounds synthesized by using microwave oven was more when compared to conventional method.

Many autoimmune disorders cause coronary involvement. Autoimmune thyroiditis (AT) is the most common cause of hypothyroidism in society. Thyroidal autoantibodies, thryroid peroxidase (TPO) and/or thyroglobulin (Tg), together with an abnormal thyrotropin concentration are usually enough for diagnois. Eighty one patients with AT, 26 with coronary artery disease (CAD) and 87 controls have been taken consecutively. Hypertensives, smokers, high body mass index having cases (>30), persons with a family history of early death from myocardial infarction (<55 years), and permanently hypertriglyceridaemics or hyperbetalipoproteinemics or persons having a low high-density lipoprotein value, even after normalization of thyrotropin concentration have been excluded. Student test has been used as method of statistical analysis. Six patients of CAD have been detected in AT (7.40%), whereas 13 cases in the control groups (14.94%) (p>0.05). One case of AT has been found in 26 cases of cad (3.84%), whereas this rate has been 13.79% among the controls (p>0.05). Additionally, positivities of TPO and/or TgAbs have been found as 36.7% and 19.23% in control and CAD groups, respectively. Although coronary involvement is frequent in many autoimmune disorders and presence of many proofs about additional autoimmune nature of CAD, we could not find a statistically significant relationship between AT and CAd. Additionally, even the six-fold higher prevalence of AT in females against the male dominance of CAd and the difference between their dominant decades may be some otherevidences of theetiopathogenetic differences of them.

Anopheles stephensi is an important urban malaria vector, which is widely distributed throughout the Indian subcontinent. The said vector species has developed resistance to various insecticides and therefore, it is desirable to develop alternative strategies including genetic methods for its control. One of the requirements for such strategy is to establish morphological mutants and to establish the genetic basis of the same. Such mutant markers could be used in the construction of genetically modified strain/s in the genetic control programme of An. stephensi. The dark colour expresses in all larval stages and pupae with full penetarence and uniform expression in both the sexes. The viability of the mutant is as good as the wild type. The genetic studies of the dark larva revealed that the gene da is mono-factorial autosomal and recessive to the wild type. The gene da is an excellent marke for An. stephensi.
